Intake Forms

It is essential that your intake forms are completed prior to your first appointment so our team can provide the most safe and effective care possible. Intake forms can be completed online any time before your appointment. If you need help completing your intake forms, please arrive 15 (or more!) minutes early for your appointment. If it's been a few years since your last appointment or you have a new ICBC or WorkSafeBC claim, you may need to complete new intake forms (we will let you know!). If you are a minor, your parent or guardian will sign the intake forms for you.

Getting Here & Acessibility

Both of our facilities are ground level and are wheelchair/scooter accessible. If you need help with the door when you arrive, please knock or ring the bell (Metrotown location). Our treatment tables are all height adjustible.

Some of our staff offer services in languages other than English - please contact us by email or phone if you are seeking treatment in a specific language. You are welcome to bring a friend or family member to help translate during your session as well.

When you arrive at the clinic, please check in with the front desk and let them know you are there for your appointment.

Our Brentwood location has free 2 hour covered parking behind the clinic. To access, turn down the driveway right next to our facility (south side of Dawson) and take an immediate right into the parkade. You are welcome to park in any stall that is not marked as "Reserved" on the wall. If you are booking back to back services and will be parked for more than 2 hours, please let our front desk know when you arrive.

Our Metrotown location has free 3 hour parking located on the west side of the building. To access from Central Boulevard, turn down the driveway right next to the pharmacy and follow the signs to the parking, which is located on your right.

Our Brentwood location is conveniently located half a block east of the Gilmore Skytrain station on the south side of Dawson street.

Our Metrotown location is conveniently located a short walk east of the Metrotown Skytrain station on the north side of Central Boulevard.

Cancellation Policy

We require 48 hours notice to cancel or reschedule an appointment. Late cancellations will be charged to the patient at 50% of the appointment cost, and no shows will be charged to the patient at 100% of the appointment cost.

What to Bring to Your Appointment

If it's your first appointment at Total Therapy, please make sure to bring a piece of government issued photo ID so that we can verify your identity.

If you haven't already put a valid credit card on file at the time of booking, please bring a method of payment to your appointment (we accept cash, debit, and most major credit cards!). In addition, if you have an active ICBC claim, WSBC claim, or extended benefits plan that you would like us to direct bill to, please bring that claim information with you to your appointment and/or include it in your intake forms.

Our therapists are happy to work around whatever you are comfortable in and will always ask your consent prior to adjusting clothing in treatment.

If you are under the age of 19, please make sure you have a parent or guardian present for your first session. They may or may not be required at subsequent sessions, which your therapist will let you know during your session.

Although not required, if you happen to have a doctor's referral or any other medical report(s) that you think may help your therapist treat you, please bring them to your appointment or email them to us before your apppointment time.
